A dermatologist at the Baylor College of Medicine in Houston is warning that laser hair removal is a surgical procedure with potential problems and consumers need to find out the experience and training of anyone they choose to perform that procedure on them. Those complications include scarring or skin pigmentation changes. And they can occur even with someone who is well-versed in the procedure. But that person should be able to deal with them better if they do occur. The arcane workings of the peer review system were the subject of the First International Congress on Peer Review in Biomedical Publication, sponsored by the American Medical Association, and held May 10-12 in Chicago. Nearly 300 biomedical and scientific editors, reviewers, researchers, clinicians, and journalists from twenty-one countries attended, including the chief editors of the worlds leading general biomedical journals, the British Medical journal, The Journal of the Americann Medical Association The Lancet, The New England Journal of Medicine, and Science. Several presentations illustrated that there are no uniform standards by which reviewers review papers and by which editors rely on reviewers to select papers for publication. For example, two reports presented conflicting evidence on whether re-review of manuscripts would alter the decisions made by editors to publish or not to publish specific papers. Another study indicated that neither authors nor academic institutions follow any set of uniform standards for manuscript preparation, submission, and publication. As a result, actions considered taboo by most editors, such as duplicate submission (submitting the same manuscript to more than one journal) and duplicate publication (salami slicing, or multiple publications reporting various aspects of the same clinical trial) may occur. Bias in peer review received much attention. While no one denied its existence and effects, debate centered on whether bias was inappropriate in all circumstances and how to minimize the biases that do cause problems. For instance, a reviewer with an obvious conflict of interest may provide the most useful review, while a reviewer free of conflict of interest may not be critical enough of a dubious manuscript. Much discussion also arose over whether double-blind review (blinding both the reviewers to the authors and institutions, and the authors to the reviewers) can reduce bias, can be equally assured for all papers and authors, or has any effect on the final decision to publish a paper. Whether peer review is designed to catch inconsistency, error, or fraud, and whose responsibility it is to regulate and correct such work fueled considerable debate among the representative gatekeepers of the leading biomedical journals, academic institutions, and government agencies. Discussants generally agreed that the purpose of peer review is not to catch fraud but to help improve those papers that are publishable (that is, with a thorough statistical review) and to weed out those papers considered not publishable. However, many attendees agreed that most papers are eventually published somewhere. The final presentation posed two of the most intriguing questions concerning peer review: How many published papers actually achieve the goal of changing the behavior of those who read them? And what happens to that rare, innovative paper that might meet this idealistic goal but is denied publication again and again because editors and reviewers fail to recognize its potential importance? In this light, is the peer review system a giant sieve with gaping holes? And are readers, those who have no interaction whatsoever with peer review, the ones who are really blinded to the process?

American First Mortgage Source Inc. will open Feb. 1 at 504 W. Mission Ave., in Escondido. Jess Chinchillas, founder of the company, recently returned to the county after serving as vice president for The Money Store, a Sacramento-based home lender. The home improvement loan is secured by a lien on the subject property, he added. Since the average home improvement loan is smaller than other kinds of home equity loans, but has just as much paperwork, the points charged are usually proportionally higher, he added.
Nursing home care in Utah is improving, the Centers for Medicare and Medicaid Services reported Wednesday. Many of those improvements were linked directly by care center administrators to a quality improvement project through HealthInsight, Medicares Quality Improvement Organization in Utah. The organization is part of the national Nursing Home Initiative, which focuses on raising standards of care in nursing homes across the country and giving consumers more comparison information.

Some compare todays coastal housing market, where prices increased the most, to the investment craze that led to the technology stock crash a few years ago. Home prices in some regions have more than doubled in the past five years, leading to concerns that the speculative bubble might burst. Homeowners in the Boston and San Diego areas saw their homes appreciate more than 75 percent during the same period. The good news is what has not gone up will probably not go down. That is, home prices in Indiana are not likely to fall with a possible drop in prices in those areas of the country that may be experiencing a bubble. So there is less risk associated with purchasing a home in Indiana compared to many other areas of the country. This also makes homes in Indiana relatively affordable compared to many other areas.

The Brian Jones Fan Club is selling tiles from the pool where the charismatic Rolling Stones guitarist was found dead on July 3, 1969. “Its not as macabre as it sounds,” said the fan clubs David Reynolds. “Its just owning something that your hero owned and touched and chose.” Each tile costs $175 and comes with a certificate of authenticity. Ten percent goes to put up a statue of Jones in his home town of Cheltenham, a genteel spa town 120 miles west of London. Since the sale began on the clubs Web site in September, 140 of the 320 blue ceramic tiles have been bought by collectors. The fund has raised $5,700 — about one-sixth the cost of the statue.

City Creek is primarily residential and retail and someday probably will accommodate additional offices. This represents a great opportunity for our city to have a resurgence again.

LAS VEGAS — Combining liposuction with Thermage–a radiofrequency energy system that heats the skin to produce collagen tightening and shortening–led to clinically significant tightening of abdominal skin at 28 weeks, compared with either treatment alone, results from a small study suggest. Combining the two procedures can be particularly useful in patients with loose skin or striae prior to liposuction, David Avram, M.D., said at the 13th International Symposium on Cosmetic Laser Surgery. This can be a way of enhancing our results in those patients. However, this was a small study. We obviously need to perform more studies with more patients. For the study, Dr. Avram and his associates treated 14 patients with standard tumescent liposuction plus Thermage (group 1), four patients with liposuction only (group 2), and two patients with Thermage only (group 3). After liposuction, patients in group 1 had four tattoo markers placed in a rectangular pattern in areas of loose abdominal skin. The area was calculated and recorded prior to treatment with Thermage, said Dr. Avram, a cosmetic dermatologist who practices in New York City.

OAKLAND — A local program that helps low-income women dress for job interviews is expanding its services and opening a mens clothing store. Wardrobe for Opportunity (WFO) invites people to donate clothing or volunteer as wardrobe consultants, inventory managers, career mentors, fundraising coordinators and office assistants. It also needs volunteers in such professional services as marketing, legal, accounting and public relations. The new Oakland mens clothing store is scheduled to open in downtown at the end of May after WFO leases a space. But I am sure you ladies have been to the cathedral.” It had been suggested to me that Palma could be the perfect destination for a mum-and-daughter break. To investigate, I enlisted the help of my mother and we set off for a long weekend of indulging our shared enthusiasms of sun, shopping, food and, of course, Gothic churches of the 13th Century. The first benefit of Palma that became obvious is how accessible it is from England - a mere two-hour flight. And, best of all, the airport is a simple 15-minute drive away from the city. So no gruelling transfers - in fact we had a driver waiting at the airport to whisk us to the hotel. It was, we agreed, an excellent start.

Squalls ripped through the southern Pacific yesterday, knocking down at least three of the 13 yachts still competing in the BOC round-the-world solo race. An American, Arnet Taylor Jnr, and two Britons, Robin Davie and Harry Mitchell, reported being knocked over. Christophe Auguin, of France, turned in another 300-mile day to remain solidly in the lead. His countryman Jean Luc Van den Heede, however, sailed a straighter course to cut Auguins advantage down to 142 miles from 239 just two days earlier.
Steve Pettengill, of the United States, sailed into Charleston, South Carolina yesterday to take second place in the 27,000-mile BOC Challenge. Pettengill, sailing Hunters Child, completed the voyage in 128 days, four hours, three minutes and nine seconds. The Frenchman, Christophe Auguin, won in 121 days, 17 hours, 11 minutes and 46 seconds.

The fact attorneys have shammed the gun industry out of millions with frivolous lawsuits, mis-represented the 2nd Amendment constantly, sued gun makers for wrong doings perpetrated by criminals who used firearms illegally, all must be a terrible mistake on their part and all easily explained away. But I understand that to generalize is always a bad idea and there must be at least one attorney who doesnt fit the category, and if so, will you write in? Of course, attorneys also wrote the Constitution, so they cant all be bad, right? But hey, before you go, did you hear the one about the attorney and the shark who, woops, sorry. Best Lawyers: Two San Diego-based lawyers at Foley Lardner LLP have been selected for inclusion in The Best Lawyers in America 2007. George L. Root Jr. represents health care providers throughout the United States, and also is listed in Chambers USA: Americas Leading Business Lawyers (2003-2006), a legal guide. The 2007 edition is based on peer-review surveys by attorneys throughout the country. Lawyers arent allowed to pay for a listing, nor is advertising accepted in the annual Best Lawyers book.
Lawyers need courage and compassion as personal virtues. But lawyers may need these virtues in ways that they had not imagined when they first entered the Bar. Statistics about lawyers are alarming. I have read that 40-50 percent of disciplinary cases involve lawyers who are suffering from an impairment, usually an addiction to alcohol or drugs. I have read that lawyers are twice as likely as other Americans to have an addiction, depression, suicidal tendencies, or stress anxiety. To be more concrete, I have read that 10 percent of Americans suffer from an addictive, emotional, or mental impairment. By contrast, these same sources estimate that 20-25 percent of lawyers suffer from an impairment. If this is true, it means that in Oklahoma at least 2,000 lawyers are practicing who are impaired. Two thousand lawyers who likely do not realize that they need to be courageous and compassionate toward themselves. Lawyers need courage to say, “I am an alcoholic,” or “I am addicted to drugs.” Lawyers need courage to say, “I am emotionally exhausted by my divorce,” or “I am overwhelmed with grief at the death of my child,” or “I am debilitated by too much work.” Once lawyers are courageous with themselves, they can be compassionate to themselves by seeking assistance to address these burdens. But not all lawyers individually will either have the insight or the courage to face the fact that they are suffering from an impairment. In those instances, their fellow lawyers must have the courage to speak the truth to the impaired lawyer. Fellow lawyers must not turn away from the impaired lawyer in the ill-advised hope that all will be OK somehow. Fellow lawyers show compassion for an impaired lawyer not by ignoring the lawyers impairment but by helping that lawyer to get needed assistance. Moreover, by acting with courage to assist an impaired lawyer, fellow lawyers can often prevent serious harms from occurring either to the impaired lawyer or to the impaired lawyers clients. Once harm occurs to clients, fellow lawyers have the obligation to report the impaired lawyer to disciplinary authorities for misconduct. Where can a lawyer with an impairment, or fellow lawyers desiring to assist, go for help? Since 1985, the Oklahoma Bar Association has supported the Lawyers-Helping-Lawyers Committee. For 12 years, the volunteer members of this committee have provided confidential, immediate responses to those who call for help. Volunteer members arrange a meeting to discuss the situation and to offer suggestions for further treatment — be it counseling, admission to a treatment center, or joining a 12-step program. Volunteer members will serve as a sponsor to meet with the impaired lawyer as often as necessary to get the lawyer on the road to recovery. Lawyers-Helping-Lawyers has the goal of recovery, not discipline. Beginning in January, the Oklahoma Bar Association committed $18,000 to match $18,000 provided by the Oklahoma Attorneys Mutual Insurance Co. to hire a part-time director for the committee. As described by the Lawyers-Helping Lawyers Committee, the director will develop treatment plans, conduct counseling sessions, make lawyer evaluations and referrals, maintain a 24-hour hotline, coordinate the activities of the committee volunteers and develop educational programs for the Bar about impairment. In addition, the director will identify professionals from other disciplines who have the skills and the desire to work with lawyers suffering from an impairment. This year is the first year that Lawyers-Helping-Lawyers has had a staff member devoted to its tasks. While the arrangement at present is only for a one-year trial period, Lawyers-Helping-Lawyers is hopeful that the trial period will convincingly show the need for a full-time director on a permanent basis beginning in 1999. Lawyers-Helping-Lawyers has acted with courage and compassion to face an issue that has not received the attention that its seriousness deserves. Lawyers are suffering; clients are at risk; the profession bears the stigma of attorneys who commit misconduct while impaired. For ourselves, our clients, and the profession, we, the organized Bar, need to say, “We, my brother and sister lawyers, are human. We are impaired. Understanding the market and recognizing that, if given a choice, families would rather have their loved ones closer to home, Andrew Tarutis, director of development at CHF during the projects design, expressed the following goals: a facility that was upscale, elegant and, above-all residential in feeling. A gracious country inn was the image that came to mind.There were good business reasons for this, as witness the following results. The facility was fully occupied in nine months, rather than the anticipated 12, has sustained a 100% occupancy rate, and continues to have a long waiting list. Administrator Kathleen Dess reports “exceedingly low” staff turnover and Nursing Director Barbara Slobin says the residents period of adjustment is unusually short.
In 1775 Elizabeth, dowager countess of Home, hired the renowned architect Robert Adam to design and complete the interiors of her newly built house at 20 Portman Square, London (Fig. 1). (1) While this patron and commission may not be among the most prestigious of Adams long career, the finished house is widely regarded as one of the best examples of his interior design genius. Lady Home was an immensely rich Jamaican heiress whose fortune increased in 1734 with the death of her first husband, the Honorable James Lawes–the son of the British governor of Jamaica. Lady Home acquired her title through her short-lived second marriage in 1742 to the impoverished spendthrift William (d. 1761), eighth earl of Home, who had abandoned his first wife two years earlier. The new Lady Home became a colorful figure, affectionately known as the Queen of Hell, (2) in the somewhat disreputable social circles of London, and she was a close confidant and supporter of her niece-in-law Anne Horton (1743-1808), duchess of Cumberland. (3) In possession of her magnificent house, Lady Home could show off to the respectable elements of London society as well as provide herself and the Cumberlands with a showcase for their anti-court social set. BRITS have always had a soft spot for Majorca. The largest of the Balearics has rarely been knocked off the top spot in the holiday chart since we discovered it as a package destination in the 1960s. Forty years on, this charming island has become a big hit with the stars too. Celebrities such as Michael Douglas and Catherine Zeta Jones have villa hideaways there. Although its best known for its sand and sea holidays, Majorca has so much to offer at this time of year too. With temperatures at a comfortable 50-60F its a perfect time for a leisurely walk in the islands rugged mountains or for exploring the quiet roads in a hire car.
Majorca is now officially the most popular holiday destination. A survey of the top 10 North East holiday destinations included Austria, Florida and Majorca, but missed out some of the regions former favourites. Resorts like the Costa del Sol and Benidorm are replaced by destinations in Malta, Greece, Turkey and Italy. And while package holidays are still top of the Geordie wish-list, more people are travelling to countries further afield. A spokesman from Newcastle International Airport said: People are starting to create their own ( booking their own flights and finding a hotel when theyre there.

A total of 91 women, aged 30-69 years, participated in the study; 50 were nonsmokers and 41 were smokers. The subjects completed questionnaires on smoking status and dietary habits and provided blood samples for nutrient analysis. Serum levels of [alpha]-carotene, [beta]-carotene, cryptoxanthin, and lycopene in smokers averaged only 71-79% of the concentrations found in nonsmokers. Dietary differences could not account for this effect. Mean serum concentrations of vitamins C and E and lutein/zeaxanthin were only slightly, and not significantly, lower among smokers, and concentrations of vitamin A were actually higher. The results for carotenoids are generally consistent with those of other studies. The lack of an association between smoking and serum vitamin C levels may reflect the relatively high vitamin C intake of this study population. The authors observe that the negative effect of smoking on serum concentrations of antioxidant carotenoids may pose a serious health risk in low-income populations already at higher risk for many chronic diseases.
This report describes six cases of hypovitaminosis D in women who had immigrated to The Netherlands from Morocco (two cases), Pakistan (two cases), Surinam, or Turkey. In all six cases, there was a long delay (7 to 103 months) between the onset of symptoms and the correct diagnosis. All of the women were subjected to multiple diagnostic tests, X-rays, and treatment with non-steroidal anti-inflammatory drugs and other medicines, four underwent extensive physiotherapy, and one with stomach pains underwent unnecessary surgery because her symptoms were believed to be due to gallbladder disease.The authors conclude that hypovitaminosis D osteopathy in women who have immigrated to Western Europe may remain undiagnosed for an unacceptably long time. Low blood levels of antioxidant nutrients such as vitamin E and the carotenoids have been observed in some older people. It has been suggested that the lack of these protective factors may increase the risk of chronic diseases in this age group. It is uncertain, however, whether low levels of circulating antioxidant nutrients are an inevitable accompaniment to aging. Plasma levels of vitamin E and most carotenoids in the elderly nuns were similar to those reported in middle-aged populations; lycopene levels, however, were lower. Plasma levels of most antioxidants did not decrease with age in the nuns; lycopene was an exception. These data suggest that, with the exception of lycopene, plasma levels of antioxidant nutrients do not inevitably decrease with aging. The relatively high antioxidant status seen in this population may reflect the fact that the sisters had regular access to a nutritious diet, with assistance in eating provided for those who needed it, and the fact that they did not smoke and seldom drank alcohol. It is possible that the low antioxidant levels reported in some other elderly population groups may reflect poor diets and/or the presence of factors which deplete antioxidants, such as cigarette smoking.
